
The Mid-Atlantic Patient Safety Organization Announces the 2026 Minogue Award Winners
The Johns Hopkins Health System Named 2026 Minogue Award Winner for Patient Safety Innovation
The Mid-Atlantic Patient Safety Center (MPSC) is honored to announce that The Johns Hopkins Health System has been named the 2026 recipient of the Minogue Award for Patient Safety Innovation in recognition of its initiative, Hardwiring Medical Device Safety: A Systemwide Model for Collaboration, Recall Management, & Equity.
The Minogue Award for Patient Safety Innovation recognizes healthcare organizations that demonstrate bold leadership and measurable impact through innovative, system-level approaches to improving patient safety. The Johns Hopkins Health System was selected for its comprehensive, collaborative model to strengthen medical device safety, improve recall management, and embed equity into device-related safety processes across the system.
“This work reflects what patient safety looks like at its best—deeply collaborative, system-designed, and focused on solving problems at their root,” said Stephanie Peditto, President & CEO of the Mid-Atlantic Patient Safety Center. “The Johns Hopkins Health System has created a model that not only improves safety within its own organization but also offers valuable lessons for health systems across the region and beyond.”
About the Minogue Awards Program
The Minogue Awards program honors a spectrum of excellence in patient safety. In addition to the Minogue Award for Patient Safety Innovation (first-place submission), the program includes the Distinguished Achievement Award (second place) and Circle of Honor for Patient Safety Excellence recognitions for additional outstanding projects. Together, these awards celebrate meaningful innovations improving care across the Mid-Atlantic region.

This year’s awardees were selected from nearly 50 submissions reviewed by a panel of healthcare experts. All winners will be recognized during a special awards reception held in conjunction with the Mid-Atlantic Patient Safety Conference in April.
Named in honor of William Minogue, MD, FACP—Maryland Patient Safety Center’s first President and Executive Director—the Minogue Awards were established by the Center’s Board of Directors to recognize Dr. Minogue’s enduring commitment to patient safety and to honor organizations making a demonstrable difference through innovation.

At MPSC, we believe the future of patient safety depends not on asking individuals to work harder, but on designing systems that make it easier to deliver safe, equitable care. We must shift toward systems that anticipate risk, support the workforce, and are built for resilience.
Too often, when harm (or a near miss) occurs, root cause analyses focus narrowly on frontline decisions or procedural lapses—missing the broader systems and design failures that allow harm to occur. Better RCAs look beyond policies and protocols to ask deeper questions: Why was the task hard to do safely? How did the design of the device, workflow, or environment contribute? Did the event depend on a staff member remembering or perceiving something in a loud/busy/distracted environment? Who was at the table when the solution was developed? The most meaningful safety improvements come when we expand the circle of responsibility—engaging not just clinicians, but also designers, manufacturers, architects, IT, engineers, and administrators in building safer care systems from the ground up.
This year’s Minogue Award sought submissions that reflect projects that hardwire safe and equitable care into systems solutions that reduce harm not by depending on perfect human performance, but by hard-wiring improvements to technology, processes, environment, and culture.

We invited healthcare organizations across the Mid-Atlantic to submit innovative patient safety initiatives for the 2026 Minogue Award. The 2026 Award winners submitted projects that:
Design safety into systems, processes, environment culture, and technologies
Reduce reliance on individual vigilance or heroism
Expand the ecosystem of safety partners beyond clinical teams
Strengthen the way we identify and respond to root causes of harm

This annual event honors the outstanding achievements of healthcare organizations who are leading the way in patient safety.
