Maryland Steps Forward: A Falls Improvement Initiative
Reducing the number and severity of patient falls. Improving outcomes. Strengthening safety across the region.

Why This Initiative Matters
Patient falls remain a leading cause of harm in healthcare facilities nationwide. Reducing the number and severity of them requires more than quick fixes; it demands structured, collaborative approaches that identify root causes and share proven solutions.
MPSC is proud to lead the Maryland Steps Forward: Falls Improvement Initiative, designed to equip hospitals and other healthcare organizations with evidence-based tools, training, and shared learning opportunities to reduce both the number and severity of falls.

How This Phase 1 Inpatient Initiative Works
Through a “train-the-trainer” model, participating hospitals:
-
Designated 1–3 site champions from a selected adult inpatient unit
-
Received education on post-fall debriefing best practices
-
Are implementing structured debriefs after falls to uncover root causes
-
Sharing findings with MPSC to inform regional learning and interventions
-
Accessing monthly implementation support, education, and peer sharing sessions
The goal is to foster a culture of safety where every fall is a learning opportunity, driving long-term improvements in patient care.

Our Shared Goals
Together, across healthcare settings, we aim to:
Reduce the frequency and severity of patient falls
Identify root causes through structured debriefing
Standardize interventions and share them region-wide
Build collaborative solutions that strengthen patient safety across healthcare organizations

By working together, we can create safer environments for patients, empower clinical teams, and and reduce the number and severity of falls for patients.
