One organization may see an isolated event. Across multiple organizations, it may be a safety signal. The Patient Safety Exchange connects people across MPSC member organizations so we can recognize risks sooner, share what we are learning, and act together to reduce repeated serious preventable safety events.

What is the Patient Safety Exchange?
The Exchange is a member-powered safety learning network. From oxygen tank depletion events to IV fluid supply shortages, it gives people across MPSC member organizations a reliable place to raise emerging concerns, ask “Are others seeing this too?”, learn how peers are responding, and determine when an issue deserves deeper learning or collective action.

Who should participate?
Anyone who works for an MPSC member organization is welcome. Patient safety does not live in one department. We want to lengthen the table to include nursing, medicine, pharmacy, quality, risk, infection prevention, informatics, technology, biomedical engineering, patient experience, operations, supply chain, frontline staff, and others who see safety risks through their work.
From Signal to Action
Not every concern needs a new project. The value is having a reliable pathway to notice, connect, learn, and decide what deserves more attention. Depending on the issue, the next step may be peer advice, shared resources, a focused workgroup, a PSO-protected Safe Table, deeper analysis, an improvement effort, or—over time—collective engagement with manufacturers, vendors, regulators, policymakers, or other partners positioned to influence the problem.
The Exchange continues between meetings
The online Exchange community on Discourse makes it easy to ask questions, continue conversations, share resources, and connect with colleagues between monthly meetings. Think of it as the Exchange between meetings—not the reason to participate, but a way to keep the network active.
